Federal Medicaid officials say the Money Follows the Person (MFP) Demonstration has helped 144,651 people leave institutions for community living since transitions began in calendar year 2008, according to a Sept. 17 CMS bulletin covering data through Dec. 31, 2025.

The brief report, CMS said, tracks how grant recipients move eligible Medicaid beneficiaries into community settings and expand support for community-based long-term services and supports. National transition counts rose 8.4% in calendar year 2024 compared with 2023, continuing an upward trend since 2022, then held nearly steady in 2025 with a −0.1% change from 2024.

Older adults and people with physical disabilities accounted for more than three-quarters of all MFP transitions in calendar years 2024 and 2025, CMS reported. MFP remains one of the main federal tools for turning Olmstead-style community living into funded transitions—not only for older adults, but for people with disabilities who still leave ICF/IID and nursing facilities into HCBS.
