Personal care agencies, home health agencies and other MaineCare providers in the state's first revalidation group must file their revalidation case by Friday, Oct. 16, 2026, or have their claims held, and those that still haven't filed by Nov. 16 will be dropped from MaineCare on Nov. 17.
The Office of MaineCare Services set out the dates in an Oct. 13 provider bulletin updating Maine's federally required off-cycle revalidation plan. The Centers for Medicare & Medicaid Services asked every state in April to revalidate its Medicaid providers over two years, starting with those considered high risk.
Deadlines for Cohorts 1 and 2
| Date | What happens |
|---|---|
| July 19, 2026 | 30-day notice mailed to affected providers |
| Aug. 17, 2026 | Revalidation window opened |
| Oct. 16, 2026 | Claims pend for providers that haven't submitted a revalidation case |
| Nov. 16, 2026 | Last day to submit after missing Oct. 16 |
| Nov. 17, 2026 | Enrollment terminated if no case has been submitted |
Cohorts 1 and 2 cover individual high-risk providers, providers without a National Provider Identifier, personal care agencies, home health agencies, hospice providers and assisted living services providers. The state's published cohort list names 182 providers in those two groups.
HCBS waiver providers are next
Maine's plan, submitted to CMS on June 5, puts home and community-based waiver services providers in Cohort 3. The cohort list shows 110 of them, split into three waves whose revalidation windows open Nov. 16, 2026, Jan. 18, 2027, and March 22, 2027. Children's adaptive behavior services follow in Cohort 4 in April 2027, and mental and behavioral health providers in Cohort 5, with windows opening from May through December 2027.
According to the plan, the off-cycle list covers about 30% of MaineCare's roughly 6,000 enrolled pay-to providers. MaineCare says it sends each cohort a notice at least 30 days before its window opens and a follow-up notice 60 days after.
Why this matters for providers and DSPs
A pended claim isn't a denial, but for a provider agency meeting payroll for direct support professionals and personal care staff, held payments hurt until the case is filed. The deadline also lands just after MaineCare skipped its Oct. 7 payment cycle.
Waiver agencies in Cohort 3 can check their window date on the state's cohort list now. Agencies have the most control over the paperwork, so confirming that ownership, location and NPI records in the enrollment file are current before the November notice arrives is the practical step.
